Requesting Proxy Access for Online Records
To request access to someone else's online medical records:
- Please visit the surgery with your ID.
- If the person is over 16, we may also need their ID.
- For children's records, we may ask for a birth certificate.
- You will also need to complete an online access form.
Once your request is approved, the linkage key will be emailed to you as the proxy holder.
